Telehealth Services
If P’AZ provides telehealth services through the Website, the following terms apply:
Consent to Telehealth: By scheduling or participating in a telehealth appointment, you consent to receive psychiatric services via electronic means (e.g., video conferencing). You acknowledge that telehealth may have limitations compared to in-person visits, including technical issues or inability to perform certain assessments.
Technical Requirements: You are responsible for ensuring a secure, private, and reliable internet connection and device for telehealth sessions. P’AZ is not liable for disruptions due to technical failures outside our control.
Confidentiality: P’AZ uses HIPAA-compliant platforms for telehealth to protect your PHI. You agree to participate in telehealth sessions from a private location to maintain confidentiality.
Arizona Licensure: Telehealth services are provided by licensed Arizona providers and are subject to Arizona laws, including A.R.S. § 36-3602 (telemedicine regulations).
Emergency Situations: Telehealth is not suitable for emergencies. In case of a mental health crisis, contact 911, the Arizona Crisis Line at 1-844-534-HOPE (4673), or go to the nearest emergency room.
Patient Responsibilities
As a user of the Website or Services, you agree to:
Provide accurate and complete information when scheduling appointments or using the patient portal.
Comply with P’AZ’s policies, including payment and cancellation policies (see Payment and Billing Section).
Notify P’AZ of any changes to your contact information or health status that may affect your care.

Payment and Billing
Fees: You are responsible for all fees associated with P’AZ’s services, including consultation, therapy, and medication management fees, as outlined at the time of service.
Insurance: P’AZ may bill your insurance provider for covered services, subject to your authorization (see our Consent to Use, Disclose & Release PHI form). You are responsible for verifying insurance coverage and paying any copays, deductibles, or non-covered services.
Payment Terms: Payment is due at the time of service unless otherwise agreed. P’AZ accepts personal checks, debit/credit cards, and HSA. Unpaid balances may incur late fees, per Arizona law (A.R.S. § 44-1201).
Cancellation Policy: Appointments canceled with less than 24 hours’ notice may incur a cancellation fee, as specified in our practice policies.
Intellectual Property Rights
This website and all content on the Website, including text, logos, images, and design elements, is owned by P’AZ or its licensors and is protected by copyright, trademark, and other intellectual property laws. Arbitration: If unresolved, disputes shall be submitted to binding arbitration in Maricopa County, Arizona, under the rules of the American Arbitration Association. You waive the right to participate in a class action lawsuit.
Jurisdiction: Any legal action not subject to arbitration shall be filed in state or federal courts in Maricopa County, Arizona.
However, we retain the right to bring any suit, action, or proceeding against you for breach of these Terms in your country of residence or any other relevant country. You waive any and all objections to the exercise of jurisdiction over you by such courts and to venue in such courts.
At P’AZ’s sole discretion, it may require you to submit any disputes arising from these Terms or use of the Website, including disputes arising from or concerning their interpretation, violation, invalidity, non-performance, or termination, to final and binding arbitration under the Rules of Arbitration of the American Arbitration Association applying.
